Will it show adult content to a family group?
No. The audience filter is applied before anything is drawn, and the family-safe set is the default. Adult prompts require an explicit opt-in, and the kids and clean sets cannot surface them at all.
Does it repeat questions?
Not within a session. The generator tracks what it has already shown you and works through the filtered pool before it starts again, so a short game will not serve you the same prompt twice.
